This information is up-to-date. Please read this message carefully as it has important
information on how you can register your child for the H1N1 VACCINE and who
we are currently able to give the vaccine to.
This information is for all of our OHIO AND INDIANA
patients:
If you want your child to receive the H1N1 VACCINE and you have not
previously registered, please
pre register your child by going to
www.h1n1vaccine.odh.ohio.gov.
No matter where your child gets the vaccine they must be registered at this
website first. If you do not have access to the
internet you will be asked to fill out a registration form before your child
can receive the vaccine.
Please read ALL information carefully:
All patients with Private Insurance will be charged a Administration Charge
of $20 per vaccine that we can bill to your insurance. Your insurance may or
may not pay this Fee. If they don't, you will be responsible for paying
this.
If you prefer, you can always receive the H1N1 Vaccine for free at your
local Health Department. The Health Department can provide these vaccines to
you free of charge because the government is paying the Health Departments
to give these vaccines.
The Government is not paying us, so we must charge the $20
Administration Charge, not for the vaccine itself but for all the time and
work that goes into giving these vaccines and all the paperwork and record
keeping that goes along with it.
If your child has Ohio/Indiana Medicaid or Caresource, you will not be
charged the $20 Administration Charge, since the Government will reimburse
us for this charge.
Again, please register your children at the above website first, if
you have not done so already.
These H1N1 Vaccine Clinic times will be open to all of our patients at our
Oxford Office only.
Your child MUST have been seen within this past year by
any of our Providers to receive either the Seasonal Flu or the H1N1
Vaccines.
