Our billing department is open from 8:00 to 5:00 Monday-Friday.

We participate in the following major insurance companies: Humana, Aetna, Cigna, Medical Mutual of Ohio, Anthem and United Healthcare. It is always a good idea to contact your insurance company and ask them if we are a provider for your plan. We file all claims for insurance companies in which we participate.

We do not bill for co-pays; you will be expected to pay all co-pays and out-of-pocket fees at your visit. For your convenience, we accept Visa, MasterCard, Discover and American Express.

Any appointment that is scheduled after 5:00 Monday through Thursday has a $30 after-hours fee attached to it. There is also a $25.00 returned check charge. We encourage you to call our billing department with any questions you may have about your account.

The parent who brings the child in will be charged for the account. In divorced situations we expect that parent to be the responsible party. We can send the other parent an itemized statement, but we cannot send out two bills.
